In the fourth and final webinar in the Middle and High School Transition series, Sonja Chatfield-Peetz, M.Ed., will discuss person-centered planning. She will specifically address:

1. What Future Planning is and how can it be used.

2. Identify common characteristics of Future Planning Mapping.

3. Identify Future Planning frames and how to complete a map.

4. Facilitation tips to complete a map.

5. Identify limitations of Futures Planning
