This Day 2 on-line training is a follow up to Day 1 - Transition to Adult Services and Accessing Community Supports. Participants must have completed the Day 1 of the training to be eligible to attend Day 2.

Upon completion of Day 1, the coaching conversation, and 1-hour virtual meeting, participants will be awarded 8 hours of continuing education certificate.
*This training is approved for 8 continuing education clock hours for Kansas licensed Speech-Language Pathologists by the Kansas Department for Aging and Disability Services.
Identify, explore and access community supports with a student in mind
Design, implement, and evaluate an implementation plan linking student education, family engagement and community supports
Understand and apply critical concepts through engagement in a virtual coaching conversation
Participate in a virtual meeting to share outcomes and reflections with colleagues providing opportunities to discuss, reflect upon, and analyze the implementation plan
